I hereby agree to the terms and tuition to participate in the programs at Standing Room Only Studios. I understand that I am liable for all fees and tuition agreed upon during registration and throughout the season as well as the due dates for all payments towards fees and tuition, regardless of my child's participation in the program. I also am fully aware and in agreement with the studio's refund policies. I am aware that after the 6 month introductory rate, my monthly tuition will increase to the appropriate monthly tuition for the program my child is placed. I give Standing Room Only Studios permission to bring my child/myself to the closest medical facility for emergency treatment and release them of any and all financial, civil, and criminal liability. I have read and understand the Liability Waiver below this form. Standing Room Only Studios Liability Waiver and Acknowledgment of Risk: I understand and agree that in participating in any dance class, workshop, rehearsal orperformance, there is a possibility of physical injury or death. I voluntarily agree,therefore, to assume all risks and responsibility for any such injury or accident, whichmight occur to me or my child during any of Standing Room Only Studios classes,rehearsals, performances, or activities. I also exempt, release, and indemnify StandingRoom Only Studios, its owners, agents, volunteers, assistants, employees, guest artists,faculty members, and/or students from any and all liability claims, demands, or causes ofaction whatsoever from any damage, loss, injury, or death to me, my children, or propertywhich may arise out of or in connection with participation in any classes or activitiesconducted by Standing Room Only Studios. I further hereby voluntarily agree to waivemy rights and that of my heirs and assigns to hold Standing Room Only Studios, itsowners, agents, volunteers, assistants, employees, guest artists, faculty members, and/orstudents liable for such damage, loss, injury, or death. I understand that I should beaware of my physical limitations and agree not to exceed them. If I am signing thiswaiver for my children, I certify that I am the parent or legal guardian and have the rightto waive these rights.Permission is granted Standing Room Only Studios to use photographs of students forpublicity purposes.
